We require a health care professional referral from someone like your Macmillan Nurse, GP, Consultant, Physiotherapist, District Nurse etc. These are sent to the Sara Lee Trust office and will be dealt with by the Therapy Manager.
You will then be contacted by the Therapy Manager who will discuss with you which therapy you would like to have and will book an appointment for you and explain more fully how the Trust can help and support you and your family.
On your first visit you will be met by your therapist who will explain more fully about your therapy.
We will offer you a number of treatments i.e. 6 in the first instance, but we seek to listen to you and your health care professionals and therefore this won’t necessarily be the end of your time with the Trust.
We make no charge for our services, in following the wishes of Sara Lee, in whose memory the Trust was established. However, you are very welcome to make a donation should you wish to do so. (www.justgiving.com/sltf).
Our aim is to offer an individual approach tailored to you and your needs. We hope to support all who are referred and to do this we may need to space your appointments out and sometimes it may be appropriate to have a break, or draw your therapies to a close, but we will always discuss this with you.
Speak to your Macmillan Nurse (or someone else from the list above) about being referred. If you are unsure, telephone the office on 01424 456 608 or e-mail sarah@saraleetrust.org.uk and we will try and help.
